Change of Name/Address/Account

When Changing Your Name

Fill in your new name in the Pension Beneficiary Application Form for Change of Name / Change of Bank and submit it to us together with a copy of your certificate of residence updated with the name or other document that evidences the name and your annuity certificate. If you are receiving your Survivors’ Pension benefits, please also submit a document clarifying the reason for changing your official registered name.
If you change your name, you must accordingly change the account holder’s name of the bank to which your pension is deposited. If you wish to change it to another financial institution, you must also apply for the change of the financial institution for depositing your pension.
With this notification, the name used for the pension benefits received from other implementing institutions (Japan Pension Service and Federation of National Public Service Mutual Aid Associations) will be also changed. (The notification is forwarded to other implementing institutions by PMAC. The time required for this process may vary depending on the implementing institution.)

When Changing the Bank Account

Fill out the Pension Beneficiary Application Form for Change of Name / Change of Bank and submit it to us together with a copy of your bank passbook’s page which contains your account details (the account holder’s name in Katakana, name of pension deposit bank, and your bank account information) or have your new bank verify your account details.

If you have changed your pension deposit bank, please notify us by the 10th of the month preceding the month of the date of pension payment.

Your pension may be paid to your old account if the processing of the form is under way. Please do not close the old account until the processing is complete and make sure that your pension is deposited to your new account.

When Changing Your Address

For Persons Whose New Address Can be Confirmed on the Basic Resident Registration Network system

If address change information can be confirmed on the Basic Resident Registration network system (hereafter referred to as "Juki Net"), individual notification to PMAC from you isn't necessary in principle.
However, it may take time until the address after the change is confirmed and registered. During the period, all mails are sent to the previous address. Furthermore, phone numbers cannot be confirmed on Juki Net anyway. When registering a new phone number, submit an address change notification using "Address Change Notification for Pensioners" as well as the case you wish to register the new address promptly.
With this notification, the address used for the pension benefits received from other implementing institutions (Japan Pension Service and Federation of National Public Service Mutual Aid Associations) will be also changed. (The notification is forwarded to other implementing institutions by PMAC. The time required for this process may vary depending on the implementing institution.)
If you need a form "Address Change Notification for Pensioners", please contact PMAC.

Note:
When the address after the change cannot be confirmed on Juki Net, "Address Change Notification for Pensioners" and a certificate of residence must be submitted.
