Maximum Amount Application/Standard copayment Reduction Certificate

The certificate is issued to the low-income to reduce their standard copayment for meal and living expenses during hospitalization. PMAC will pay High-cost Medical Care on low-income category to the medical institution, which is normally paid later as a benefit payment, thus you don't have to pay the cost of the care at the hostital.

Issuance

Submit a “Application form for Maximum Amount Application/Standard copayment Reduction Certificate” together with a document that proves low-income through your school office.
For those exempted from municipal resident’s tax, My Number can be used to verify the member’s exempted status instead of attaching a Tax Exemption Certificate for Municipal Resident’s tax.

Note

Persons who belong to the low-income category need the certificate to use the scheme at a medical institution even when he/she has an elderly recipient certificate (70 years old or older). The certificate expires annually as of the 31st of July. To be qualified, you must renew the certificate each year by submitting an official low-income evidence paper.
