Documents Required for Application for Approval

In addition to the “application form for dependency approval”, documents that can prove the relationship (from a member’s side), that the parties are living in the same household, and the financial dependency relationship must be attached.

The contents of the following documents can be verified using Individual Number Card (“My Number Card”), so principally, you do not need to attach them.

• Resident Card (only if the insured is the head of the household)
• Income Certificate (Tax Exemption Certificate for Income) (only if you had no income for the past three years)
• Employment Insurance Separation Notice (1) (2)
• Family register (only if the spouse, child, or parent were living with the insured in the same household)
