This certificate is required for the Maternity Benefit after expiry of your membership. You are entitled for the Direct Payment System when you had been a member for a year or more and gave birth within six months of membership expiry.
Submit a claim form(arbitrary) with your membership number (the number given to your Membership Card), your name, seal, your birthdate, the expiry date of membership, the expected date of delivery, your address, and your telephone number in it. A sample claim form is shown below. The certificate will be sent directly to your address.
Childbirth beyond six months of membership expiry is not accepted. When you do not use the Direct Payment System, you are requested to submit the application form for the benefit.
