Maximum Amount Application Certificate

This certificate reduces the amount paid at hospitals for members or dependents (under age 70, or age 70 and above) whose copayment rate for the Elderly Recipient Certificate is 30% (restricted to individuals with a Monthly Standard Salary between 280,000 and 790,000 yen). Of the self-paid amounts at medical institutions, PMAC pays the amount equivalent to “Benefit for High-cost Medical Care” directly to the medical institution. The certificate will display the income category of the member.

Issuance

When you anticipate high medical bills (high copayment with insurance covered), submit a “Claim form for Maximum Amount Application certificate” in advance through your school office. The certificate will be issued to you in about 10 days after request through your school office.

Note

The income code may be altered when there is a change in your Monthly Standard Salary. A new certificate with a revised income category will then be issued. Upon receipt of a new certificate with a revised income category, return the old one to us. Even if you had to pay high-cost medical care expenses in full at the medical institution because you did not have the certificate with you, you will be automatically reimbursed later so that you will not claim to high medical fees or any other disadvantages.
If the certificate reaches its expiration date and you wish to continue using it, update procedure will be required approximately two weeks before the expiration date.
