Health check-up expences subsidy

Comprehensive health checkup system expenses will be subsidized.

Eligibility

Members 35 years or older (including Voluntary Continued Insurance Members), their dependents, and faculty members 75 years or older as of the date of comprehensive health checkup system

Comprehensive health checkup system eligible for subsidies

Subsidies are granted for expenses paid directly to a medical check-up facility for a comprehensive set of standard examination items.

Points to note

  • Subsidies will be granted only for a comprehensive set of standard examination items. Please confirm it with your medical check-up facility.
  • Subsidies will not be granted for regular medical check-ups that schools conduct as part of their welfare program because they are not subsidiary activities for schools.
  • The Specified Health Check-up voucher for Member's dependents and Voluntary Continued Insurance Members is not applicable comprehensive health checkup system. Please return the voucher to us if it remains unused.

Medical check-up facilities

  • Among medical institutions offering comprehensive medical examination services, the Tokyo Rinkai Hospital Medical Health Center and most of contract medical check-up facilities across Japan offer the services at a discount.
  • The list of medical check-up services facilities is available on our PMAC website with a facility search engine (in Japanese only).
  • Note that comprehensive health checkup system on a mobile examination unit is outside the scope of subsidies.

How to apply

(1) Directly operated / contract medical check-up facilities

Make a reservation with a medical check-up facility and tell them that you are a PMAC member. At the counter, you will be asked to present your Membership Card, Membership Card for Dependent, Membership Eligibility Certificate, or Welfare Facility User Certificate.

Note:
The examination fee may differ between online reservation and reservation by telephone.

(2) Other medical check-up facilities

When you make a reservation with a medical check-up facility, confirm with the facility that they offer a comprehensive set of standard examination items. Subsidies are granted for a comprehensive set of standard examination items only.

How to invoice expenses

To apply, submit the Application Form for Subsidies for Comprehensive health checkup system through your school. The form is available at the mutual department of PMAC or our Garden Palace. It is also downloadable from our website.

Subsidies will be remitted to your school’s subsidy receiving account.

Note that Voluntary Continued Insurance Members must submit the form directly to PMAC. Subsidies will be remitted to the subsidy receiving account registered when you became a voluntary continued insurance member.

Note1:
Members' dependents, Voluntary Continued Insurance Members, and their dependents 40-74 years in the current fiscal year must also submit the following documents together with the form: the Medical Check-up Results Entry Sheet or a copy of the Comprehensive Medical Check-up Results Report and the Standard Questionnaire. Incomplete documents will be returned for resubmission in principle.

Note2:
If the Application Form for Subsidies for Comprehensive health checkup system does not come with an evidence of receipt of payment, please send the original receipt separately issued by the facility together with the form.
